What do I bring with me when applying for a passport?

For new passport applications, please bring the following:

• Completed Form DS-11, in black ink only

• Passport photo (we do not take photos in office; you may try the local Walmart. Please click here for photo requirements.)

• Original or Certified Copy of your Birth Certificate (hospital birth certificates will not work)

• Photo Identification

• 2 methods of payment

          • 1 will be to the Clerk of the District Court. This method can be check, money order, or exact cash.

          • 1 will be to the U.S. Department of State. This method can be check or money order only.

                    • Fees may vary based on selected options. If your payment needs to be filled out in advance, please contact our office for

                       exact amounts.

The above items are for our most common passport applicants. If you are unsure of what to bring with you, please contact our office at (308) 254-2814. If the name on your application does not match the name on your photo identification, you will need to bring in documentation of your name change, such as your original or certified copy of your marriage license, or certified copy of your name change decree.
